Each form can be edited as needed. The patient’s most recent audiogram is automatically placed on the New Order, Ear-Mold and Repair forms.

• Each field title can be changed by editing the blue title area.
• Each drop-down list can be modified to present your desired options.
• You can print one or two copies of each form.
• You can email or fax your form to the appropriate contact at your suppliers location.
• Click the Home button when finished. Your form saved in the Supplier Portal history for future reprinting or review.
